There's false positives like post #10246707. While that's an argument for any hyper-specific tag, panty_bulge seems rather useful.

It's probably necessary because it removes the very common scenario of a girl in panties and a guy having a bulge in the same image. panties bulge -trap -futanari -crossdressing still has some ~2k results. Of course outside of resolving that specific issue, it is functionally nothing more than a two tag search in usage. For example panties bulge futanari still becomes panty_bulge futanari to find images of futanari and panties bulge -trap crossdressing_(mtf) becomes panty_bulge -trap crossdressing_(mtf) to find images of non-otokonoko/trap male characters wearing panties, etc.
