There's probably some sort of sharp hidden object in that tissue.

Ah, I think I know what Tewi's doing... she's taking a tissue and twisting one corner of it to make it thinner. Then she poked the pointed end into Reisen's nose. My cousin used to do this to me when I was younger, except he would poke it into my ear. It tickles like hell, and trust me, it's not fun.
