I notice that Remilia got ill in just the same way as Sakuya and Patchouli. I guess this means that here, Remilia and Flandre are moroii (living vampires), rather than strigoii (dead vampires)? Although it might depend on what was meant by the distinction in Romanian legend...Occasionally, it seems like what was meant was whether or not they were visible!

I think the answer is "neither" - they're a species of akuma (thus the Scarlet Devil Mansion) with some of the traits of Western vampires (blood drinking, sensitivity to sunlight) but not others.
I even think difficulty with running water isn't part of their official ZUN canon, but instead was introduced by fanon authors. I'm pretty sure there's no official canon on their vulnerability to human illness, though I know of at least one doujin that plays with the concept.
Colonel Aki's Scarlet sisters, of course, don't even have all of their canon traits if it violates the Rule of Funny.
