Kuso doesn't mean "crazy" in this context. It's a pretty typical abusive/weak swear-like phrase that is presumably derived from the homophonous colloqualism for feces, and is used like how in English we say "Damned" or "Fuckin'" or "Shitty" something like that, but to my understanding it's a bit milder. So "stinkin'" fits. Just as "damned" isn't saying the person being described is actually cursed to eternity in hell, stinkin' doesn't mean foul-smelling here, though the connection between this use of "kuso" and feces is, the connection between feces and a foul stench makes sense.

And going with "dyke" over "lesbian" is appropriate because "rezu" is derogatory in Japanese. "Lesbo" might be a close cognate, but I think "rezu" is more derogatory, and the strength of "dyke" makes up for any vulgarity lost by translating "kuso" to "stinkin'".
