As much as this Kogasa wants to be better at surprising people, I think she'd be heartbroken if the human children avoided her out of fear. She's just too kind-hearted to be truly mean.
I think just about anything can be made cute if you make one change to it; make it smaller. Hell, I once called a tiny spider kinda cute while driving. And thats saying something.

I couldn't find much, myself (mostly under ドクロ回し), but I saw enough hits on the phrase to accept it as a phenomenon unto itself, even if I still don't know the details.
It sounds like that trick where you put a round object on top of your umbrella, towards the edge, and spin the umbrella quickly so that the object bounces up and down while seeming to remain in one place. Only, in this case, the round object is a skull rather than a ball.
Anyway, that's what immediately popped into mind, and I choose to picture this imagery regardless of whether or not it turns out I'm wrong, if only because it's funny.

It's actually pretty brilliant: She can consistently give them pleasant surprises, and also surprise people who find out how much human children like her.
