Ultra Vires is a a legal term, indicating that something has been done outside of legal authority. It probably isn't the best term to use here, as it's not a legal matter, but an authority one.
It generally is seen in corporate law matters, such as where a companies agent operates outside the scope of the companies constitution, which can render any contracts that he entered into ultra vires, and thus voidable. (The opposite in intra vires). This could be seen if the companies constitution forbid entering into a loan, but an officer of the company used their authority to do so with a third party.
This only applies to organisations with narrowly-defined objectives; as such it is really only seen in non-profits and incorporated societies, as corporations are generally charted to do any and all actions that align with the corporation's purpose.
It is not used in a military context as far as I know.
Also, this is Commonwealth law, I have no idea about the US or Civil law situations.

Insubordination is the direct refusal to follow an order. It's also a different word in Japanese from 越権行為 which refers directly to exceeding authority. (不服従 is insubordination)
