Well Kaga could be off-duty and I really don't think that the shitty Admiral that tried to force Kashima is the only admiral on duty. If I remember correctly the Kaga of this artist is under the command of Doraemon.

Also, like Shounen Fake pointed out, she could notice that his voice is very familiar (and, incidentally, way of speaking; English isn't exactly good for illustrating the subtle naunces of Japanese speech patterns).

Casts, gisps are pretty much common medical supply, unless they are specially made, which seems unlikely with what I see, I don't figure how she'd notice.

For the voice thingy, yes, I found it rather unlikely he'd speak to her or anyone near her when he's trying to be just some strager eating, which is why I said "-I guess- -if- he talks". At best he'd talk to order something, but he's already eating stuff.
