She has elongated earlobes. Jizo statues (what her appearance is based on) have ears like that, and a lot of statues of other Buddhist figures as well. "Kṣitigarbha" is the article for them on Wikipedia.

ZUN actually said the stage 4 boss is creepy, though some people think that's a mistranslation and he was referring to something else about her that's creepy, not the design.
