1) That ... I'd prefer it to be a by-case affair, but separate notes for the examples you cited is better. (If the first two parts of the Inazuma bubble you cited contained an entire sentence instead of entire sentences, those two can probably be merged. Kaga's is a lost cause due to the bubble layout, though, even if it somehow contains only a single sentence.)
2) Probably leave them as is. Especially if it involves smth like unusual verbal tics (in this series this usually singles out Inazuma, though Goya and Kuma are another examples I can think of right off the bat that Reiji currently employs) or a rogue few katakana at the end of an otherwise normal sentence lacking unusual tics (guess who, deesu~).

Oh, to answer the actual question: I prefer the variable option, the one that has the least amount of note protruding from the speech bubble. Typically, that would be one note per bubble in cases like Kaga's multibubble.
