In the original sentence "really" is written in katakana (vaguely similar to capitalized words in English), so leaving it out doesn't seem right.
Moreover, the phrase "potato masher" doesn't quite capture the model of stick grenade specified under the furigana, so it probably should be spelled out in the translation.
