Amiya is indeed technically the leader but based on a few glimpses, Kal'tsit can technically overrule her. That said, in terms of chess, the king is the one that needs protecting and generally stays outside of combat like the Doctor, who is usually being protected. Since this image is supposed to mirror the parent, it would make more sense for Amiya, both a leader and combatant, to be queen. Not to mention all the speculation about Amiya being royalty or perhaps possessed by royalty
Perhaps it's symbolizing that, as much as the Doctor may use the operators as chess pieces, Amiya is still essential and her survival is non-negotiable to them.

I also like this interpretation but I do find it funny that Amiya has a skill that kills herself.
According to what Operational Intelligence has hinted at, the old Doctor (before amnesia) sees all of Rhodes Island operators as nothing more than chess pieces, too.
This just means he sees Amiya as nothing more than a Queen, where even though she's our strongest piece, she's nothing more than a means to an end.
