Besides, if you look in the original MoF artwork, you can interpret how Suwako looks as either Suwako being a young girl, or as Suwako being a tomboyish peasant girl (simple clothing, straw hat) which simply has no secondary sexual characteristics because ZUN is allergic.
The fanarts made Suwako firmly cemented in the "loli" category, even though it doesn't seem that's what ZUN intended, possibly because, much like Shiki Eiki and Komachi, fanartists have a thing for drawing characters that come in twos as having one be One Head Taller, and Kanako drew the "Adult and buxom" straw, while Suwako drew the "Lolified" straw.

With Shikieiki, it's more of a exaggeration for sake of contrast (She's pretty tall with a modest figure, but still pales to her amazon co-worker) Suwako could have simply assumed a younger looking form as to just enjoy herself. That and in 12.3 her mannerisms are on the cutesy side mostly. It's safe to say during the olden days, Suwako had a form similar to this.
