That girl showed up in Tomoya's dreams when he was in high school, so she should be just a symbolic premonition thing or something. In other words, not really because she showed up during his high school life (despite who she actually is).
I just wonder if all that running actually did kill Nagisa's dad though.

Seems like you didn't watch Clannad After. Because the girl_from_the_illusionary_world (and this is a big spoiler from Clannad After) is Nagisa and Tomoyo's daughter Ushio and the doll is Tomoyo himself, both of them being in one of the alternate universes that Kotomi's parents were investigating. So this is pretty clearly a time paradox indeed.

I was referring to Ushio but the girl from the illusionary world started out as a dream when Tomoya was in high school. So I thought girl_from_the_illusionary_world would have better fit during the time they were in high school rather than the actual kid herself post-timeskip.
