I wonder how many cat girl images I've looked at over the years that had both cat and human ears and I was never noticing because the cat ears draw all the attention.

So far, the only franchise to actually depicts both animal ears and human ears is Kemono Friends. My guess on the reason why they did that is that they wanted them to look more "human-like". Though this leave the problem of which ear is actually used, or did they actually hear a different frequentcy from both ears?

It's also the limitations of the modeling software they were using that had human models but they couldn't model the ears out.

Plenty of other franchises do, as well. It's just a matter of both effort and attention. People just plain don't notice.

Arknights, for instance, canonically has only the two leftmost.

Kaenbyou Rin has stood out years as an example of a character with both. Recently we have Azur Lane's Atago as well.

Also, in Strike Witches you have characters who grow animal ears but retain their human ears when using their magic.

A lot of franchises do, actually. Azur Lane and RWBY, for example. Even Touhou has at least Rin having the same thing.

It's fairly common in animal earred vtubers as well, I'm pretty sure like half of the animal girls in Hololive have alts that show two sets of ears
