Shouldn't the one punching and the one receiving the punches be reversed? Especially with Kotomine specializing in punching and Shirou specializing in swords poking out of his skin to be almost like an armor (like Armstrong's NANOMACHINES)?

Actually yes. the final confrontation in Heaven's Feel is from Shiro's perspective but it's also Kotomine punching a literal human made of swords.
