in the novel she is described as almost flat, yet on all official art that has her with revealing enough clothing she has medium boobs. Them again the big ones in Mushoku are really big, so i can see, in universe, they considering medium to be small.

Even if her clothing somehow deletes three cup sizes, she's wearing the same outfit here as in all those official artworks, so it should still be deleting those cup sizes.

It's fanart. It's not uncommon for artists to tweak a detail or two while maintaining the overall appearance of the official character.

If it were one artist, I wouldn't notice, but they're alllikethis. You'd think there would be some DFC lovers... but I guess they all go for Roxy, and are trained not to care about Elinalise's archetype. Even when both are in the same (lewded) picture, they make Elinalise oversized. And any of the ones that artist commentaries refer to her as "slutty" or "bitch" (in the Japanese sense of meaning "whore") or "horny" while doing it. It's like how twintails on a tsundere was really popular after the original Fate came out and Tohsaka Rin became the standard-bearer of the trope, and people demanded twintails to be a "real tsundere", it's like they're not accepting a thirsty older blonde woman character if she isn't stacked.
And to point to a different character just to show how consistent Elinalise's portrayal is, Ghislaine gets a lot more variation on how ripped she is, and musclewomen are a much more divisive concept than DFC.

I had no idea. I just chalked it up to this artist wanting large breasts but drew the ones who have those from behind so they just improvised. But now that I think about it, even if that was the case, it feels like an odd choice when it would've been just as easy to give Roxy bigger boobs.
