Given which side of the door she standing on, my bet is that she was ready to do funny stuff to a sleeping doctor.

That looks like she already done something with doctor and right now looking at Amiya.

Going by other images, the doctor's room is labeled on the inside for some reason.

I’m assuming she stepped in on Nian and Doctor based on the artist’s previous work, maybe.
