Similar thing happened to me. Pawn went on a tantrum decided to punch a mortar loaded with an antigrain shell. Needless to say, they died. Very painfully.
At least it wasn't as embarrassing as one raider I had who overdosed on go-juice upon drop podding onto my colony and died before firing a single shot.
Nah, all their limbs got blown off and they bled out in horrific agony within a couple hours... Along with a couple other colonists nearby. I reloaded a save and took better care of my mortars after that... And also nuked that colonist a dozen times in devmode for catharsis.
