It definitely happens in China, where I live. Japan and Southeast Asian countries, I've only been there for leisure, but I've seen it there too.

Hell, smoking around kids happens all the time. In the South (where I reside) old restaurants had smoking sections the drifted into the non-smoking.

And on the other hand, Mokou is a health nut because she cannot, by definition, be unhealthy; Aya's and Akyuu's sources only indicate that she's cagey. It simply happens that Smoking Delinquent Mokou (totally baseless) is more charming than Health Nut Yakitori Mokou (probably lies). Which I'm happy with, because I can then imagine her as Sakamoto Maaya voicing girl-Kujo Jotaro.

Haha, I live in the South East Asian region, and well, I've been exposed to the smoking environment ever since I was 5. Was playing some Road Rash 2 with my grandpa while my dad was with me. Both were smokers at that time (They have long since quit smoking. :D), and the room would be filled with the cigarette smell. I didn't mind it at all o.O
