Luffy looks like he has a right fist to me, it's just that it's hard to see because of what appears to be speed lines around the wrist and up the forearm. It has a bit of a blurring effect and ends up taking away from the shape that would be there, effectively making it look like he just has a long forearm, no wrist, and no fist.
However, I do think you're right about Sanji's other leg. It definitely doesn't look like it was drawn in anywhere. Same thing for Zoro actually.
But if we've got Sanji doing his flaming foot gimmick (I fear I've forgotten the specific name) and Luffy using Gear Second, shouldn't Zoro be doing his nine-swords thing to match?

You're right, but it was possibly because drawing 3 heads, 6 arms and 9 swords for a character in a picuture like this, would be far too complicated.
