I´ve always wondered about this. There must be more Hakurei people than just the shrine maiden, like in the village or something. If she dies before having descendants they can just get another girl from a branch family to do the job or manage something.
Sanae on the other hand...Maybe some tengu or kappa can take her place?
But no worries because main character death is taboo. At the very least, Reimu is safe.

Maybe he was chosen by Yukari since it has something to do with the border, but meh, I don't think that he will be important in the story so, I'll stop thinking about that
