I think its an autopistol whose design resembles a bolt (pistol?) for good luck some other superstition. Still, that means this commissar REALLY got the short end of the stick: the commissars who can't afford to start out with a bolt-weapon usually get a las-pistol instead. At least he still got his chainsword.

The artist could've be influence with the Ciaphas Cain potrays (Despite never being mentioned of using a boltpistol in his life, the artist tends to draw him using it as the book cover. There was even once where he was draw dual wielding them.)
