They're all quotes. I found a transcript of the movie on some site, did quick translations of the text, then searched for the corresponding lines ("Smiling" was the only difficult one, since the Japanese only mentions Dave "getting better", but not at what or how).
Presumably, the original text is also made up of quotes from the Japanese translation of the film, but since it's never even occurred to me before to try and watch it in that language, I can't really say for sure.
