Its appearance can vary with the artist, however she is generally depicted as a 17-year-old JK (high school girl) with grey hair, animal_ears and a tail, wearing a school uniform, a white serafuku, white thighhighs, and a red scarf.
According MoeGirl Encyclopedia, the origin of this trend is attributed to a thumbnail drawn by MKiiiiii for one of his Dark Souls II gameplays back in 2014.
