ZUN said himself that Nitori does not have a bald spot although there is one there. And those are gills on her neck.
Well, Perfect Memento says that Kappa have a plate on their heads and a shell on their backs... but Zun seems to have dropped that. Not like we had any kappa characters when that book came out.
I kinda take it to be kinda literal actually(as well as misinformation). Perhaps Akyuu met a Kappa with a literal plate on his head and she figured they all did. Ofcourse, having a plate on his head wouldn't mean he had a bald spot. That's like accusing someone of wearing a hat because he has a bald spot.
