You are aware Remilia has no idea (like most of Gensokyo) that Reimu is pregnant.

It could be that she isn't pregnant, but it would have to be a case of intentional misdirection by the author.
I'm BETTING on her being pregnant, and that during childbirth, the power of the mother gets transfered, or mostly transfered, to the child. Or even during development.
I'll say this moment that I don't hate this doujin. I just find Remilia's reaction to all of this extremely pointless, unless her desire from the start is to kill Reimu... But I still don't see why. Just to be evil? I don't get it. Maybe the rest of this doujin will tell us more?
She almost killed Marisa out of sheer impulse and is attacking the shrine while completely losing her mind. There is no way she couldn't have known what is going on. The events of this story have been happening for quite a while now, so she must know of the plot already.

It's an alternate reality, so the author is free to modify and interpret characters in different ways in his own universe. Well over half of the doujins on this website are obviously and blatantly non-canonical. That's how the Touhou Project fandom works. Deal with it.

Eh, Remilia is a spoiled brat at heart. Without reading further pages, I guess that what happened was: Remilia gets progressively angrier that Reimu is largely absent. This culminates in she bringing the entire SDM crew on a formal attack against the shrine. Yukari scrambles to get people interested in keeping Gensokyou's existance for the defense. (Keine and Marisa, even if Marisa seemingly doesn't know much)
Remilia likely knows about as much as Marisa, which is just about nothing. It's the distance, secrecy, and lack of details or reason for being kept out of the loop that hurts and causes them to act out.
Marisa and Remilia really had two different reactions based on the relationship they have with Reimu. Marisa passively backed away and emo'd about.
On the other hand, to Remilia, this is the one who defeated the great, charismatic, Scarlet Devil. I've just been guessing at all this, granted. But where Reimu's distance stabs Marisa in the heart, it wounds Remilia's stubborn pride and massive ego to the point of anger that the fabulously talented miko who took her down and affected her so much is now subservient and resigned to her fate as a Hakurei, never to fight her or anyone ever again.
Of course, the end now is probably that both Marisa and Remilia just wanted to be there for Reimu in her time of need, but were manipulated against each other by Yukari, especially on this night, who believed they would be a distraction to her duties.
