Because one of us didn't check in time to see if that line was already translated. Notes are't updated in real time, only on a refresh, so one of us hit translate, and while he was translating, the other one did the note, saved it, and this didn't show up on the first translator's screen, so he saved another note.
This has been corrected now, thanks to clamz.

Yeah, what ThunderBird said. That seems to happen a lot with the more popular comics, especially Mizuki Hitoshi's. I've occasionally seen even 3 translation notes on one bubble.

This has been bugging me for a while, but is KoKo incapable of closing her mouth?
