It's not all that disturbing. It's just... a face, specifically just the eyes with what might be a single (bloody?) tear track and maybe the tip of an ear on the right.

Personally, I had to pull the thing into an image editor and lighten it manually. Turning up my screen to painful brightness wasn't enough.

And nobody cares that Koishi's third eye is gouged out?

People are too busy caring about how the darkness is staring back at us.

If you have an LCD monitor then you can try observing the picture from the top-down. You should be able to see it then.

Ignoring the face for a second, Koishi/Yukari is really quite stunning in this picture.

Only the head (and limbs maybe) is Yukari's. The rest is still Koishi.
