Like...sometimes there's something right in front of you and you won't notice unless someone points it out at times. I think everyone experienced that at least once in their lives.

The game Omake mentions that she stayed out of sight from everyone by acting "unconsciously".

Essentially, she moves through people's "unconscious". Their unconscious would notice Koishi, but it is the part of people's brains that they are not aware of, so they are actually not aware of Koishi either.

Basically this. Your senses percieve her, but only the part of your brain that you regularly ignore (the unconcious/subconcious) recieves the information. Your concious mind somehow doesn't register her or disregards her until she does something that forces your attention on her.
