It might need some tag gardening, but that assessment is incorrect - the tag is unisex, and only applied to >5000 posts.
Yea I'm not sure about that. For example, a quick look at 1girl solo large_breasts cowboy_shot (to roughly filter out the posts where there's no waist shown) is almost indistinguishable from the tags in this implication.
I'm not saying the tag should be substituted with a 4 tag search BTW, just that in the posts where there's a waist, it's always noticeably narrower than hips, except for maybe some loli posts. It's the whole point of the average anime girl anatomy.
-1. The slender waist tag has no wiki, while the narrow_waist has a specific wiki. Given that slender waist has some 3k posts and nearly no overlap with the almost 1k posts for narrow waist, I think there is a high chance of adding noise to the narrow waist tag by migrating all those slender waist posts. I would suspect that a large portion of the slender waist images do not meet the requirements for the narrow waist tag.

Agree on this.
If both are the same concept but one is more extreme, shouldn't narrow_waist implicate slender_waist? Just like very_long_hair implicates long_hair.
Maybe rename narrow_waist -> very_narrow_waist and alias slender_waist -> narrow_waist, then create implication very_narrow_waist -> narrow_waist. This would be consistent with other very_* tags.
